Rehab on the Move - Physiotherapy

Rehab on the Move Physiotherapists are trained to assess and treat a variety of conditions that affect your physical function, in the comfort of your own home.

Physiotherapists are often referred to as “movement specialists” as they are very focused on how you move, be it getting in and out of bed, up and down your stairs, walking, jogging, or even propelling your wheelchair. By having a Rehab on the Move Physiotherapist visit you at home you will be able to practice your mobility skills in your own environment as well as alleviating your need to travel.

Who We See
Our team of experienced Physiotherapists specialise in providing rehabilitation and therapy to people:

  • Following a stroke or traumatic brain injury
  • Following a spinal cord injury
  • Following muscular skeletal injury (sports injuries and work related sprains and strains)
  • Following surgery (neurological, cardiac, amputation, orthopaedic and joint replacement)
  • Living with a neurological disability (Cerebral palsy, multiple sclerosis, Parkinson's disease)
  • Experiencing balance and mobility difficulties related to age
  • Requiring reconditioning following hospitalisation or illness

Assessment
At your first visit your Physiotherapist will assess you in order to identify your strengths and weaknesses. Your Physiotherapist will use a combination of standardised assessment tools as well as observing your functional mobility.

Your Physiotherapist will record baseline data on your abilities at this initial assessment in order to be able to track change as you progress with your Physiotherapy Program.

Establishing your goals
Following your Physiotherapy assessment you and your Physiotherapist together, will determine your rehab goals. It is important that you are working towards goals that are important to you!

Your individual physiotherapy program

An individualised Physiotherapy program will be devised for you to work towards achieving your rehab goals. Your program may involve the use of balance, strength and co-ordination activities. You may be given a home exercise program to work on in between your Physiotherapy appointments. Your Physiotherapist will provide education on correct movement techniques if required as well as educating carers if applicable.

Rehab on the Move Physiotherapists travel with a range of equipment to ensure that all your physiotherapy needs are catered for at home.

If particular equipment is required to assist you such as a walking aid, splint or orthotic your Physiotherapist can arrange for you to trial the equipment before purchase. With your consent your Physiotherapist will liaise with your other health professionals to ensure a coordinated team approach to your recovery.

Evaluating your Physiotherapy Program

At each session your Physiotherapist will be taking note of your progress and will upgrade your program as improvement is made. In the event that progress anticipated is not being achieved your Physiotherapist will review your program and make changes to try and get the right results, it is no use persevering with something if improvement is not being seen.

Your progress will continue to be monitored until your goals are reached.

Case Conferencing
A very special part of your behind the scenes treatment is Rehab on the Move’s case conferencing. This is an opportunity for your Physiotherapist to discuss with other team members your program and progress. This provides the opportunity for your therapist to liaise with other highly qualified professionals to ensure the highest standard of treatment.

Rehab on the Moves team includes: Physiotherapists, Occupational Therapists, Speech Pathologists and great Administration Staff.
